Graceconnections provides non-emergency medical transportation in areas defined by our contracts with transportation brokers and health plans. Eligibility, trip types, and service levels follow each payer’s program rules.

Primary focus: Denver metro and surrounding Colorado communities—confirm exact counties and ZIPs with your broker agreements.
Modalities and availability (e.g. ambulatory, wheelchair-accessible) depend on what is authorized for each trip through MediRide or your plan.
Coverage is not changed or extended through this website; your broker or health plan confirms where rides can be scheduled.
