For members & families

If you need a ride to covered medical appointments, your health plan or transportation broker arranges eligible trips—often through MediDrive. Graceconnections provides the ride once authorization is in place.

  1. Contact MediDrive or your plan

    Call MediDrive at (303) 398-2155 or toll-free at (855) 489-4999—or use the transportation number on your member ID card or plan materials. MediDrive confirms your eligibility and schedules authorized trips.

  2. Have details ready

    Appointment address, date and time, mobility needs, and any escort or equipment requirements they ask for.

  3. Day of the trip

    Be ready at the agreed pickup window. If something changes, call MediDrive at (303) 398-2155 or toll-free at (855) 489-4999—not this website.
